Character Certificate issued by the Principal of the institution last attended.

Caste & Income Certificate

Reserved category candidates must bring the original Caste Certificate from D.M. and father’s Income Certificate.

Anti-Ragging Undertaking

Anti-Ragging Undertaking duly signed by the candidate and parent or guardian.

Compulsory Service Bond

Affidavit Bond on ₹100 stamp paper for compulsory service of 2 years after completion of the course; otherwise ₹10 lakh is payable.

Transfer & Migration Certificate

T.C. and M.C. of the concerned Board/University.

NEET Admission Affidavit

Affidavit regarding admitted NEET/not admitted on ₹10 stamp paper.

Self-Addressed Envelopes

Five self-addressed envelopes with a ₹5 stamp affixed on each envelope.

Domicile Certificate

Domicile Certificate, wherever applicable or required.

Aadhaar Card

Candidate’s Aadhaar Card for identification and admission records.

Other State Certificate

Applicable to candidates who passed High School/Intermediate outside the state, as per the admission brochure.

MANDATORY DECLARATIONS • UNDERTAKINGS • SUPPORTING DOCUMENTS

Affidavits, Undertakings & Supporting Documents

Submit all prescribed affidavits and undertakings, duly signed and notarised as required, according to the latest admission notice.

Compulsory Service Bond Candidates must submit a compulsory service bond affidavit on ₹100 stamp paper for two years of mandatory service after completing the course. Failure to fulfil the service obligation may attract a ₹10 lakh penalty.

Anti-Ragging Undertaking Candidates must submit the official anti-ragging undertaking duly signed by both the candidate and their parent or guardian.

NEET Admission Status Affidavit Candidates must submit an affidavit on ₹10 stamp paper declaring their NEET admission status and confirming whether they have secured admission at another institution.

Self-Addressed Envelopes Candidates must submit five self-addressed envelopes, each bearing a ₹5 postal stamp.
