Affidavit Bond on ₹100 stamp paper for compulsory service of 2 years after completion of the course; otherwise ₹10 lakh is payable.
T.C. and M.C. of the concerned Board/University.
Affidavit regarding admitted NEET/not admitted on ₹10 stamp paper.
Five self-addressed envelopes with a ₹5 stamp affixed on each envelope.
Domicile Certificate, wherever applicable or required.
Candidate’s Aadhaar Card for identification and admission records.
Applicable to candidates who passed High School/Intermediate outside the state, as per the admission brochure.
Compulsory Service Bond
Candidates must submit a compulsory service bond affidavit on ₹100 stamp paper for two years of mandatory service after completing the course. Failure to fulfil the service obligation may attract a ₹10 lakh penalty.
Anti-Ragging Undertaking
Candidates must submit the official anti-ragging undertaking duly signed by both the candidate and their parent or guardian.
NEET Admission Status Affidavit
Candidates must submit an affidavit on ₹10 stamp paper declaring their NEET admission status and confirming whether they have secured admission at another institution.
Self-Addressed Envelopes
Candidates must submit five self-addressed envelopes, each bearing a ₹5 postal stamp.
